Good Faith Estimate Notice

Under federal law (the No Surprises Act), you have the right to receive a "Good Faith Estimate" of expected charges for non-emergency health care items and services.

Oasis Chiropractic PA dba Oasis Chiropractic & Wellness Center is committed to price transparency. If you are uninsured or not using your health insurance benefits to pay for services (self-pay), we are required to provide you with a written Good Faith Estimate (GFE) of the total expected charges before you receive scheduled non-emergency care.

Your Rights Include: You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs such as office visits, chiropractic adjustments, examinations, therapies, diagnostic tests, equipment, supplies, or any other reasonably expected services.

We will provide the Good Faith Estimate in writing: At least 1 business day before a scheduled service or item (if scheduled at least 3 business days in advance).

Within 3 business days of your request or if scheduled with less notice.

You can ask us—or any other provider—for a Good Faith Estimate before scheduling an item or service.

If your final bill is at least $400 more than the Good Faith Estimate for any provider or facility involved in your care, you have the right to dispute the bill through the federal patient-provider dispute resolution process.

Save a copy or photo of your Good Faith Estimate for your records—it is important if you need to dispute charges later.

The Good Faith Estimate is not a contract and does not require you to obtain services from us or any listed providers/facilities.

 

Actual charges may vary due to changes in your care plan, unforeseen complications, additional medically necessary services, or other circumstances. The estimate is based on information known at the time it is provided.

This notice applies only to uninsured or self-pay individuals. If you are using insurance, different rules (such as Advanced Explanation of Benefits) may apply.
