The Most Common Sentence I Hear About the Thyroid
"My thyroid labs are normal — so my doctor says my thyroid is fine. But I don't feel fine."
If you've said some version of that, this article is for you. Thyroid testing has become common, and that's genuinely a good thing — many of my patients in Cottage Grove walk in already knowing their TSH number, and some even know their thyroid antibody numbers. People are more informed than ever.
But knowing your number is only the beginning of the story. The bigger questions are: what does that number really mean, what else should we be looking at, and most importantly, what's actually stressing your thyroid in the first place?
That last question is where thyroid nutrition testing comes in — and where my approach at Oasis differs from just reading a lab and moving on.
Let me walk you through how I actually look at the thyroid.
First, the Numbers: "Normal" and "Optimal" Aren't the Same Thing
If you've followed my writing, you know one of my core themes: there's a difference between sufficient and optimal. Nowhere is that clearer than the thyroid.
TSH — thyroid-stimulating hormone — is the number most people know. It's the signal your brain sends to your thyroid, and it's the standard starting point for thyroid testing. Here's where it gets interesting:
The conventional medical reference range for TSH typically runs from around 0.4 up to roughly 4.0 or 4.5. So if your TSH is 3.8, you're "in range," and you'll often be told your thyroid is normal.
But many functional and holistic practitioners work with a tighter optimal range — commonly around 1.0 to 2.0, with closer to the lower end often considered better. Why the difference? Because the wide conventional range is built as a statistical average of a general population — one that includes plenty of people with undiagnosed thyroid issues. And there's real research behind the tighter view: even within the "normal" range, higher TSH levels have been associated with things like higher cholesterol. So the argument is that "just inside normal" and "truly optimal" may not be the same place.
Now, I want to be straight with you, because honesty is how I earn your trust: this is a genuine, ongoing debate in the medical world, not settled science. The functional optimal range is a clinical framework that many practitioners find useful — it is not a formal diagnostic cutoff, and conventional endocrinology still anchors diagnosis to the standard reference range. Both lenses have value. The reference range is essential for diagnosing actual disease. The tighter optimal range helps explain why someone can be "normal" on paper and still feel far from their best.
I read the number both ways — once to respect what conventional medicine is screening for, and once to understand your symptoms.
Beyond TSH: The TPO Antibody Story
TSH alone doesn't tell the whole story, which is why I also pay close attention to TPO — thyroid peroxidase antibodies.
Here's what that test is really telling us. Antibodies are part of your immune system. When TPO antibodies are elevated, it can be a sign that the immune system is involved with the thyroid — the hallmark of an autoimmune thyroid condition.
And this matters enormously, because the majority of patients I see have already been diagnosed with Hashimoto's — the most common form of hypothyroidism (an underactive thyroid), and an autoimmune condition, meaning the immune system is part of the picture. Many of these patients arrive already knowing their TSH, and increasingly their TPO antibody numbers too, and often already working with their physician on thyroid medication.
So let me be very clear about my role here, because it's important: I don't diagnose thyroid disease, and I don't touch anyone's thyroid medication — that's between you and your physician. If your doctor has you on thyroid medication, keep working with them on it. My job is different, and it's the part that often gets left out.
The Question Almost No One Asks: What's Stressing Your Thyroid?

Here's the gap I work in.
Conventional thyroid care is very good at measuring the thyroid and, when needed, replacing the hormone it's not making enough of. What it often doesn't ask is: why is the thyroid struggling in the first place? What's putting stress on it?
That's the whole focus of thyroid nutrition testing at Oasis.
